Description

Help students practice filling out forms while building daily living skills and working on functional reading. This FREE independent living skills activity is designed for teens and young adults working on life skills for real-world situations.

Students help Jordan apply for an adult library card by using realistic source documents to locate the information they need and transfer it accurately to a library card application. Students work with a sample Michigan identification card, proof of address, and personal notes—just like they may need to do when completing forms in everyday life.

This activity works well for adulting made easy activities and can also be used as part of a transition curriculum, independent living curriculum, functional life skills classes, or special education life skills programs.

Instead of practicing isolated skills, students apply information in a meaningful real-world situation while practicing attention to detail, reading for information, and accurately completing a practical form.
